Abstract
Benzodiazepines remain the gold standard for catatonia treatment. Reserve antipsychotics for catatonia with prominent psychosis interfering with care or after exhausting other treatment options. Clozapine appears to have the best safety and efficacy profile among antipsychotics, especially when catatonia follows clozapine cessation. When using antipsychotics, choose low-potency agents, start at minimal doses and increase slowly, and always co-administer benzodiazepines to mitigate risks.
