Abstract
BACKGROUND: Benign prostatic hyperplasia is a common condition in older men, often causing significant symptoms that negatively impact quality of life. Due to the aging population, this condition is becoming increasingly important from a medical and social perspective. Surgical interventions, particularly endoscopic ones, remain the primary treatment option when drug therapy is ineffective, as they are less invasive and can be used in patients with comorbidities. However, choosing the most appropriate endoscopic method for elderly patients requires consideration of their clinical characteristics and potential risks of postoperative complications, highlighting the need for a comparative evaluation of various surgical approaches to achieve optimal outcomes
AIM: To compare clinical efficacy and safety of different endoscopic surgical methods for the treatment of benign prostatic hyperplasia in elderly patients.
METHODS: The study included elderly patients with clinically and instrumentally confirmed benign prostatic hyperplasia who underwent endoscopic surgical treatment. Patients were divided into groups depending on the surgical technique used. All patients underwent standard clinical and laboratory examinations, ultrasound assessment of prostate volume and postvoid residual urine, and evaluation of lower urinary tract symptoms. Statistical analysis was performed using parametric and non-parametric methods; p <0.05 was considered statistically significant.
RESULTS: Endoscopic surgical techniques led to a significant reduction in lower urinary tract symptoms, improvement in urodynamic parameters, and decrease in postvoid residual urine. Differences were observed in operative time, duration of catheterization, and complication rates between the compared methods.
CONCLUSION: Endoscopic surgical treatment of benign prostatic hyperplasia in elderly patients is effective and safe. The choice of technique should be individualized based on patient’s characteristics and disease severity.
