Abstract

<jats:p>Orthognathic surgery corrects skeletal discrepancies of the jaws, with profound functional and aesthetic impact, but the result perceived by the patient is mediated by the soft tissue envelope that covers the new bone base. Orofacial harmonization, recognized as a dental specialty, offers minimally invasive resources capable of refining this envelope. This article analyzes the integration of orofacial harmonization in the planning and aesthetic outcome of orthognathic surgery. An integrative literature review of a qualitative nature was adopted, articulating the classic framework of soft tissue facial analysis, the evidence on the accuracy of three-dimensional virtual planning, the methodological approach of injectable treatments and the normative framework of the specialty. The results demonstrate that facial aesthetics results from the sum of two planes, the bone base and the soft tissue envelope, and that comprehensive facial analysis should guide planning from the beginning. Three-dimensional soft tissue simulation shows good overall accuracy, but lower precision in the lips, chin and paranasal region, precisely the areas in which orofacial harmonization resources, such as botulinum toxin and fillers, find a complementary role after stabilization of the surgical result. However, integration requires hierarchy: orofacial harmonization refines, but does not replace skeletal correction in true deformities, and camouflaging severe discrepancies constitutes an ethical and technical limit. It is concluded that the planned integration of the two approaches enhances the quality of the aesthetic result, provided it is ordered by correcting the base first and refining afterwards.</jats:p>
