Abstract
<jats:p>The oncology safety of medication is a complex issue that is dynamic and constantly changing, and it is tied to swift therapeutic innovations and further complications in the system. The narrow therapeutic index, toxicity, complexities of the treatment process, and growing drug formularies are the typical features of oncology drugs, making them even more harmful to patients in the case of an error. These risks share the most common one, the look-alike, sound-alike (LASA) medication error, which has long been a major hazard that has been repeatedly brought up in the history of different generations of cancer therapy. It has been demonstrated that the LASA risk is not unique to a particular group of drugs or a period of history but rather is a continuum driven by the development of drug-naming conventions, conventional practices involving the use of drugs, and clinical working processes.</jats:p>
