Abstract
<jats:p>Введение. Причинами врожденной дуоденальной непроходимости чаще бывают пороки развития самой кишки. Антенатальная диагностика атрезии двенадцатиперстной кишки (ДПК) возможна с 14–19-й недели гестации. Частичная непроходимость ДПК вызывает наибольшую сложность в диагностике из-за позднего проявления клинической картины. Цель. Изучение результатов диагностики и лечения дуоденальной обструкции у новорожденных и поиск путей совершенствования ранней диагностики при частичной непроходимости ДПК. Материалы и методы. Исследование является ретроспективным, рандомизированным, одноцентровым. Проведен ретроспективный анализ медицинских карт стационарного пациента у 29 новорожденных (12 мальчиков и 17 девочек) с непроходимостью ДПК с 2011 по 2025 г. в учреждении IIIA уровня (БУ «Сургутский окружной клинический центр охраны материнства и детства»). Результаты. За 15-летний период в университетской клинике различные виды дуоденальной непроходимости зарегистрированы у 0,022% (n=29) новорожденных. Гестационный возраст составил 33 нед.+6 дн.±5 нед.+3 дн. Масса тела при рождении – 1902 [700; 4080] г. Полная непроходимость ДПК в виде атрезии диагностирована в 55,17% (n=16) случаев, что сопоставимо с данными P. Puri, M. Hollwarth [1]. Диагноз был установлен антенатально у 44,8% (n=13). Сопутствующий синдром Дауна подтвержден у 20,7% (n=6) детей; врожденные пороки сердца – у 31,1% (n=9). Всем новорожденным с диагнозом атрезии ДПК после подготовки выполнена операция дуодено-дуоденоанастомоза по Kimura. Частичная непроходимость ДПК при мальформации кишечника с фиксацией купола слепой кишки над duodenum врожденными спайками (синдром Ледда) выявлена на 2–3-и сутки в 27,6% (n=8) случаев. Операцию Ледда выполнили в 7 случаях. При завороте кишки и явлениях острой мезентериальной ишемии у 1 ребенка выполнена резекция кишки и наложена энтеростома. Диагноз мембраны ДПК выставлен у 10,3% (n=3) пациентов на 5–7-е сутки жизни. Операция заключалась в дуоденотомии, иссечении мембраны ДПК с вариантом дуоденопластики (подобно пилоропластике по Гейнеке – Микуличу). В 6,9% (n=2) случаев установление редких причин стеноза дистальной трети ДПК произошло лишь к 3-й неделе жизни пациентов. Оперативное лечение этим детям выполнено в сроки 25–28 суток жизни. В группе пациентов с дуоденальной непроходимостью летальности не было. Заключение. Антенатальная диагностика обструкции ДПК возможна с 19-й недели беременности. Частичная непроходимость ДПК вызывает наибольшую сложность в диагностике. У новорожденных с клиническими признаками частичной дуоденальной непроходимости обосновано применение современных методов медицинской визуализации, прежде всего ультразвукового исследования (УЗИ), эзофагогастродуоденоскопии (ЭГДС) и контрольной рентгенографии брюшной полости при пассаже контрастного вещества, с целью определения уровня обструкции ДПК.</jats:p> <jats:p>Introduction. Congenital duodenal obstruction is most often caused by malformations of the intestine itself (atresia, membrane, stenosis – internal narrowing of the lumen). Antenatal diagnosis of duodenal atresia is important and possible from 14–19 weeks of gestation. Partial duodenal obstruction poses the greatest diagnostic challenge. First of all, potential malrotation with the risk of midgut volvulus requires immediate imaging examination, including barium enema. Stenosis, or compression of the duodenal lumen from the outside, can occur in cases of altered normal topography of surrounding organs and vessels. In early infancy, diagnosis of partial duodenal obstruction is difficult and may be delayed until food intolerance becomes more evident. In these cases, examination should include ultrasound, esophagogastroduodenoscopy (EGD), and control abdominal radiography with contrast medium to determine the level of obstruction. Purpose. Studying the results of diagnosis and treatment of duodenal obstruction in newborns and searching for ways to improve early diagnosis in case of partial obstruction of the duodenum. Materials and methods. This study conducted a retrospective analysis of data on the clinical types of duodenal obstruction, diagnostic features, and treatment in 29 newborns from 2011 to 2025 at a level IIIA institution (the Surgut Regional Clinical Center for Maternal and Child Health). Results. Over a 15-year period, various types of duodenal obstruction were registered in 0.022% (n=29) of newborn patients at the university clinic; gender distribution was 12 boys : 17 girls. Gestational age was 33 weeks+6 days±5 weeks+3 days. Birth weight was 1902 [700; 4080]. Complete duodenal obstruction in the form of atresia was diagnosed in 55.17% (n=16) of cases, which is comparable with the data of P. Puri, M. Holwarth (2009). The diagnosis was established antenatally in 81.25% (n=13). Concomitant Down syndrome was confirmed in 37.5% (n=6) of children; congenital heart defects – in 56.25% (n=9). All newborns diagnosed with duodenal atresia underwent Kimura duodenoduodenostomy after preparation. Partial duodenal obstruction due to intestinal malformation with fixation of the cecum dome above the duodenum by congenital adhesions (Ledd syndrome) was detected on days 2–3 in 27.58% (n=8) of cases, of which 1 patient had a midgut volvulus with acute mesenteric ischemia. The diagnosis was based on the combined clinical presentation of partial duodenal obstruction and signs of incomplete intestinal rotation detected by barium enema. The Ledd procedure was performed in 7 cases. Due to intestinal volvulus and acute mesenteric ischemia, resection and ostomy were necessary in 1 child. Endoscopy and radiographic contrast techniques were used. Duodenal membrane was diagnosed in 10.34% (n=3) of patients between days 5 and 7 of life. The surgical solution consisted of duodenotomy, excision of the duodenal membrane, and duodenoplasty (similar to Heineke – Mikulich pyloroplasty). In 6.89% (n=2) cases, rare causes of distal duodenal stenosis were identified only by the third week of life. These children underwent surgical treatment between days 25 and 28 of life. There was no mortality in the group of patients with duodenal obstruction. Diagnosis of partial duodenal obstruction is difficult in the neonatal period. Examination should include ultrasound, esophagogastroduodenoscopy, and control abdominal radiography with contrast medium to determine the level of duodenal obstruction. Conclusion. Antenatal diagnosis of duodenal obstruction is possible as early as 19 weeks of pregnancy; detection of this pathology can be an indicator of severe hereditary syndromes (Down syndrome, cardiac septal defects, etc.). The variability of the causes and severity of congenital duodenal obstruction requires different approaches to providing surgical care to these patients. Partial duodenal obstruction poses the greatest diagnostic challenge. The combination of the clinical picture of partial duodenal obstruction and radiographic signs of incomplete intestinal rotation indicates a high probability of midgut volvulus and is an indication for emergency laparotomy. In newborns with clinical signs of partial duodenal obstruction, the use of modern medical imaging techniques, primarily ultrasound (US), esophagogastroduodenoscopy (EGD), and control abdominal radiography with contrast medium infusion to determine the level of duodenal obstruction, is warranted. Further randomized trials should be conducted to investigate the causes of partial duodenal obstruction using medical imaging techniques.</jats:p>
