Abstract
<title>Abstract</title> <p>Purpose To report a case of a bilateral acute angle closure associated with isatuximab, an anti-CD38 monoclonal antibody increasingly used across multiple lines of therapy in multiple myeloma. Results A 41-year-old woman presented to the emergency department with bilateral primary angle closure hours after isatuximab infusion. Anterior segment optical coherence tomography demonstrated ciliochoroidal detachment in both eyes. Due to associated ocular hypertension, triple topical antihypertensive therapy was initiated, achieving normalization of intraocular pressure. Clinical symptoms improved within two days, and complete resolution of the uveal effusion was observed eleven days after drug administration. Conclusion Isatuximab, currently approved not only for relapsed or refractory multiple myeloma but also in earlier-line combination regimens, may be associated with drug-induced secondary angle closure due to uveal effusion. Given its comparatively lower global use compared with other anti-CD38 antibodies such as daratumumab, this potential adverse event may be underrecognized.</p>
