Abstract
<title>Abstract</title> <p>Objective To evaluate the correlation and agreement between the Visual Prostate Symptom Score (VPSS) and the Arabic International Prostate Symptom Score (IPSS) in Egyptian men with lower urinary tract symptoms (LUTS), and to compare the two instruments in terms of ease of administration, completion time, and patient preference. Materials and Methods This prospective study enrolled 400 male patients aged ≥ 40 years presenting with LUTS at Tanta University Hospital's Urology Department (April 2024–April 2025). All patients completed both the VPSS and Arabic IPSS questionnaires alongside standard clinical evaluation, including history, physical examination, laboratory tests, uroflowmetry, and transabdominal ultrasonography. Correlation between scores was assessed using Spearman's coefficient, with agreement evaluated by Bland-Altman analysis and weighted kappa statistics. Results A total of 400 men with a mean age of 60.9 ± 9.91 years were enrolled. The median total IPSS was 15 (IQR 8–20.75) versus a median total VPSS of 7 (IQR 4–9). VPSS and IPSS total scores showed a statistically significant positive correlation (rs = 0.645, p < 0.001), as did their quality-of-life subscores (rs = 0.504, p < 0.001). Agreement in symptom severity classification was substantial (weighted kappa = 0.71; overall concordance 82.8%). Significantly fewer participants required assistance completing the VPSS than the IPSS (18.5% vs. 34.8%, p < 0.001), and mean completion time was shorter for the VPSS (1.9 ± 0.76 minutes) than the IPSS (4.7 ± 1.85 minutes, p < 0.001). The majority of patients (67.0%) preferred the VPSS over the IPSS (23.5%), with 9.5% expressing no preference. Conclusions VPSS demonstrates strong correlation with IPSS in Egyptian men with LUTS and offers practical advantages in terms of ease of administration and reduced need for assistance. VPSS represents a valid alternative assessment tool, particularly in populations with varying literacy levels.</p>
