<title>Abstract</title> <p> <italic> <bold>Introduction:</bold> </italic> <italic>Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy and often presents with nonspecific symptoms such as fever, fatigue, and musculoskeletal pain. These manifestations may overlap with common infectious diseases, particularly in rural settings where infections are highly prevalent and access to specialized diagnostic services is limited. Consequently, the diagnosis of ALL may be delayed when clinical features are initially attributed to concurrent infection.</italic> <italic> <bold>Case Presentation:</bold> </italic> <italic>We report the case of a child from a rural area who presented with prolonged fever, pallor, and reduced activity. Initial evaluation suggested an infectious process, and empirical antimicrobial treatment was initiated. Despite treatment, the patient's symptoms persisted, accompanied by progressive anemia and thrombocytopenia. The overlapping clinical and laboratory findings created significant diagnostic uncertainty and delayed consideration of an underlying hematologic malignancy.</italic> <italic> <bold>Diagnostic Assessment and Intervention:</bold> </italic> <italic>Further investigations revealed abnormal peripheral blood findings, prompting bone marrow examination. The bone marrow study demonstrated lymphoblast proliferation consistent with acute lymphoblastic leukemia. Following confirmation of the diagnosis, the patient was referred to a pediatric hematology-oncology service and initiated on standard induction chemotherapy according to institutional protocols.</italic> <italic> <bold>Outcomes:</bold> </italic> <italic>Clinical improvement was observed after initiation of leukemia-directed therapy, with resolution of fever and gradual recovery of hematologic parameters. The case highlights the importance of reassessing the diagnosis when presumed infection fails to respond to appropriate treatment and when persistent cytopenias are present. Acute lymphoblastic leukemia confirmed after referral and bone marrow examination at a tertiary hospital.</italic> <italic> <bold>Conclusion:</bold> </italic> <italic>This case underscores the diagnostic challenges of ALL in children presenting with concurrent infection, particularly in resource-limited rural settings. Early recognition of warning signs such as unexplained cytopenias, persistent fever, and lack of response to antimicrobial therapy may facilitate timely diagnosis and improve clinical outcomes.</italic> </p>
