Abstract

<title>Abstract</title> <p> <bold>Background:</bold> Dyslipidemias are common in Chronic Kidney Disease (CKD) patients and are associated with an increased cardiovascular risk, which is the main factor of morbidity and mortality among these patients. To enhance the management of CKD patients, preventive measures against cardiovascular morbidity may be adopted, and their lipid profile before dialysis may be documented. The objective of this study was to analyze variations in lipid parameters among patients with CKD at a hospital in Burkina Faso (West Africa). <bold>Methods:</bold> A case-control study was conducted from April to September 2025 on 120 subjects, shared out into two groups. Lipidic parameters were measured, and p-values were calculated in SPSS to investigate associations between lipidic parameter variation and independent variables, with a significance threshold of 0.05. A Student's t-test was performed to compare lipid parameter means between the two groups. Parameters with a significant p-value (&lt;0.05) in univariate analysis were subsequently subjected to multivariate analysis to identify risk factors. <bold>Results</bold> : A total of 120 subjects were included, comprising 59 cases of CKD and 61 controls. Hypercholesterolemia was more frequent in the CKD group than in the controls, at 30.5% and 27.9%, respectively. A significant increase in LDL-c, non-HDL-c, and the atherogenic index was observed in the CKD group, with p-values of 0.006, 0.026, and 0.01, respectively. A significant decrease in HDL-c levels was observed in the first group (p=0.013). Hypertriglyceridemia was more frequent in the CKD group (22%) compared to the second one (9.8%). The frequency and severity of lipid abnormalities were proportional to the stage of CKD, except for hypercholesterolemia, which was inversely proportional to the stage. Multivariate analysis confirmed the association between hypoHDL-cholesterolemia and diabetes (p-value=0.013), hypertension (p-value=0.012), and both diabetes and hypertension (p-value=0.008). <bold>Conclusion</bold> : This study demonstrated and characterized lipid abnormalities in patients with chronic kidney disease (CKD) who were not yet on dialysis. These abnormalities were associated with an atherogenic risk and were proportionally correlated with the stage of CKD, excluding hypercholesterolemia. </p>
